The purpose of the website is as follows: SlimForLife is developing a digital service where patients receive diagnosis and treatment from doctors and other specialists such as dietitians and personal trainers.
3.OWNERSHIP OF THE WEBSITE
The website is owned and managed by SlimForLife as a legal entity based in Romania.
4.USER TERMS
To use our service, you must log in to SlimForLife.online. To receive adequate care and advice, you must then truthfully answer questions about your health, diseases, current symptoms, and medications on the website, as in a regular doctor’s visit. In some cases, you may also need to go and take blood tests at one of the sampling units we collaborate with. If you suffer from acute illness during your care with us, you must contact another healthcare provider and seek emergency care. It is also the user’s responsibility to contact SlimForLife if this occurs.
CONSENT
I understand and accept that a team of professional caregivers, including doctors, dietitians, personal trainers, and coaches, may collaborate to provide comprehensive and holistic care for my health and well-being. I am aware that each member of the care team will handle my personal information with complete confidentiality and in accordance with applicable laws on patient privacy and data protection. I consent to these caregivers sharing relevant information and communicating with each other to coordinate my care and optimize results. When I register an account with SlimForLife, a patient record is created, as I contact a registered caregiver as a patient. Whether I proceed with investigation and treatment or cancel, SlimForLife has a statutory requirement to record the contact.
YOUR RIGHTS
You have the right to withdraw your consent regarding the processing of personal data based on your consent. As we are a healthcare provider, we are legally obligated to retain most of your health data. Upon your request, all health data that we are not legally obligated to retain will be deleted. To update, correct, or delete information we have about you, exercise rights granted to you as above, or contact our data protection officer, please contact us at support@SlimForLife.online.
PRIVACY POLICY
By accepting these user terms, the user consents to the processing of personal data as follows: user’s name, email, social security number, and health condition.
Personal data will be processed for the following purposes: all information we collect becomes part of your medical record. All information is classified as confidential according to medical records law. Your answers and results will be treated as a medical record so that unauthorized persons cannot access them.

User’s personal data will not be disclosed to third parties unless required by mandatory rules or government orders.
e) Period for which personal data will be stored
Personal data may only be stored for as long as necessary for the purpose.
f) Encryption
The website has an SSL certificate (Secure Socket Layer) that ensures the secure and confidential transmission of users’ personal data by encrypting the information.
g) Users’ rights
Data portability: In some cases, a person who has provided their personal data has the right to have it transferred to another service, for example, a media service. The first recipient of the personal data must facilitate such a transfer.

USER BREACH OF AGREEMENT
A user who violates what is stated in these user terms will be handled as follows: the account will be terminated immediately.

Slim For Life, the owner of the SlimForLife.online website, disclaims the following legal responsibilities: SlimForLife is integrated with third-party applications and services (“third-party applications”) to make the service available to you. These third-party applications may have their own terms of use and privacy policies. You understand and accept that Slim For Life does not endorse and is not responsible for the behaviour, features, or content of any third-party application. For the terms, including this point, Romanian law shall apply. Disputes arising from the terms shall be settled by Romanian courts. The customer has the opportunity to provide feedback on the care directly to Slim For Life.
JURISDICTION
The SlimForLife.online website is operated from Romania, and the user terms are governed by Romanian law.
